The Impact of Gatekeeping: A Candid Discussion
Presented by Dr. Yuko Higashi
With insight gained from over 25 years of experience working with trans* communities and through service as a founding board member of a professional society for transgender care in Japan, Higashi will present a brief history of the progress and struggles of a “GID Nation,” a society that has developed and implemented support systems for trans* centered around the medical concept. While gatekeeping by “professionals/experts” has had a significant effect on trans people’s lives and well-being, there are new concerns over a movement for the exclusion of trans* by those describing themselves as “gender-critical.” This is another form of gatekeeping which may have a serious impact on the future of care/support systems, especially for trans children and youths. She will share her concerns with these recent developments and reaffirm the importance of a human rights-based approach.
Families Supporting and Advocating for their Underage Transgender Child

Most of us are familiar with the journey of a transgender person to be able to access hormone therapy that would allow them to affirm their gender, however in the case of transgender youth, often times their voices are not heard and even when their family supports them they are not able to get the desired hormone therapy in a timely manner. In addition to this, a lot of countries still demand in their gender identity laws that the individual undergoes “some sort” of medical intervention in order for them to be able to change the gender marker in their birth certificate which would allow them to reflect their true gender. We would provide examples of laws from different countries in Latin America and how the parent’s role in advocating for their children has become a key element in the process for their child’s wellbeing.
Gatekeeping For Your Own Good?
Presented by Prof. Esben Esther Pirelli Benestad
Based on empirical scientific data, extensive clinical experience, contact with organisations working for the welfare of individuals who experience gender incongruence and experiences from being a transgifted person themself, Benestad will give answers to the following questions:
- What newer empirical data are to be found that supports gatekeeping? - Meaning papers that convince both clients and peers that the gatekeeping really is for your own good?
- Do we have data to serve insights to the clients’ satisfaction with a gatekeeing ward?
- Do we have data on experiences of discrimination in society as a whole and in health care systems in particular?
- To the extent that gatekeeping postpones or dismisses therapeutic assistance, what might be the consequences?
- What newer empirical data are to be found that support gender affirmative approaches based on informed consent?
- Is that which we experience in Norway representative for the consequences of gatekeeping?
Benestad’s geographical viewpoint as a clinician is Noway, since they receives clients from all over that country. Norway has one ward at the Oslo University Hospital (OUS) that has had and still has a monopolized national position in diagnosing and treating persons who experience gender incongruence. This ward operates by very strict gatekeeping, hence we can build an empirical image of some consequences of gatekeeping.
